I just want to add a few more questions to the second part of below:
4. How many nationally reportable incidents were reviewed?
5. How many complaints were reviewed?
6. How many ombudsman reports were reviewed?
Please confirm receipt of the amended request. For clarity this is now the request?
1.    Has the NHS Delivery Unit been asked to conduct any sort of investigation into maternity / obstetric services at Swansea Bay University Health Board?
2.    Has the NHS Delivery Unit been asked to review any Serious Incident Reports maternity / obstetrics / neonatal from Swansea Bay University Health Board?

If yes to either of the above please provide:
1.    The date the review was started and completed.
2.    The terms of reference for the review.
3.    A copy of the completed review
4.    How many nationally reportable incidents were reviewed?
5.    How many complaints were reviewed?
6.    How many ombudsman reports were reviewed?

Thank you for your recent request dated 12th September 2023.
The NHS Wales Delivery Unit has not been asked to conduct any sort of investigation or review into maternity/obstetric services at Swansea Bay University Health Board.
The NHS Wales Delivery Unit has not been asked or commissioned to review any incident reports specifically in relation to maternity/ obstetrics/neonatal services from Swansea Bay University Health Board.
